Congressman Eric D. Singson Scholarship Grant (CEDSSG)

This program aims to provide financial assistance to deserving incoming college students who belong to ICC/IPP communities from all towns and city in the 2nd District of Ilocos Sur.

Instructions: Read General and Documentary Requirements. Fill in all the required information. Do not leave an item blank and always check your answer before moving to the next question.

 

QUALIFICATIONS:
1. Filipino citizen and a resident of the 2nd District of Ilocos Sur.
2. Must be a member of Indigenous Peoples (IP) group.

DOCUMENTARY REQUIREMENTS as per AO Number 5, Series of 2012

1.0 SCHOLARSHIP APPLICATION DOCUMENTS: (Neatly filed in a long white folder)

  • 1.1       Certificate of Confirmation (COC) of Tribal Membership from NCIP
  • 1.2       Duly filled-up Scholarship Application Form with 2×2 ID Picture (2 Original copies)
  • 1.3       Application Letter addressed to Congresswoman Kristine Singson
  • 1.4       Certified Photocopy of Birth Certificate
  • 1.5       Certified Photocopy of Good Moral Character from School Principal/Guidance Counselor
  • 1.6       For High School Graduate: Certified Photocopy of High School Card, or DepEd Form 138-A (2 Photocopies)
                 For College Students: 
                Certificate of Final Grades with School Seal and Signature of School Registrar for the last semester attended. 
                (1 Original & 1Photocopy)
  • 1.7       Certificate of Residency Indigency issued by the Barangay Captain (1 Original & 1 Photocopy)
  • 1.8       Recommendation from the Municipal/City Mayor
