Medical Assistance thru Expanded Assistance to Individuals in Crisis Situation (E-AICS) THROUGH DSWD
Medical, Hospitalization, Medicine or Burial Assistance
How to avail:
1. Visit our Facebook page – facebook.com/TulongPusoSerbisyo.
2. Send us a message with your name, address and contact number.
3. Upload the following documents:
✓ Final Hospital Bill / Medical Prescription / Laboratory Request / Funeral Contract,
✓ Medical / Death Certificate,
✓ Original Certificate of Indigency of the claimant,
✓ Photocopy of 1 valid ID of the claimant.
4. You may approach your Barangay Officials to help you in sending the required information and documents.
5. Wait for a call from our District Staff to guide you on how the assistance will be delivered to you.
** IMPORTANT REMINDERS **
✓ Please make sure that all the required information and documents are complete.
✓ All the submitted information and documents will undergo a detailed review and evaluation by out district staff.
✓ Only qualified clients will be be given assistance and those submitting fake documents will be prosecuted with the full extent of the law.

Medical Assistance for Indigent Patients (MAIP) through the Department of Health (DOH)
How To Avail:
1. Final hospital bill
2. Medical certificate
3.Certificate of the indigency from barangay captain name of patient.
4. Email claimant’s name, address and contact number together with pictures of the required documents to maip@kristinesingson.ph
